Once your application is ready to go

• Attach previous ethics committee approvals to the application
   
• Ensure the head of your institution (or a person senior to the Principal Investigator) has signed the AIHW confidentiality undertaking
   
• Ensure all relevant members of staff have signed the AIHW confidentiality undertaking
