This report summarises the process through which Primary Health Networks and the Australian Institute of Health and Welfare collaborated to implement a proof-of-concept general practice data demonstration project. Using dementia as the focus, the project explored data sharing, governance, analytics, and the functionality of the Primary Health Insights platform. The report highlights the project's key findings, challenges, and opportunities, emphasising the complexities of primary health care data in Australia. Insights from the project will guide future efforts toward developing a national primary health care data collection.
