Describes adverse events reported in three routinely collected data sets in Australia, and offers suggestions for further assessment and improvement of the data collections to allow the occurrence of these conditions to be gauged more accurately. The working paper should prove a useful contribution to deliberations on improving health information systems to measure adverse events and monitor the efforts to reduce them.
