Safe and high-quality health care depends on the sharing of health information between health-care providers. In 2016, nearly 1 in 4 patients reported inadequate information sharing between the emergency department they visited and their General Practitioner (GP) or usual place of care. This report looks at gaps in information sharing between providers, using the 2016 Survey of Health Care.
