• The indicators under Measures 4.6 and 5.5 attribute data to the incorrect place of residence. Data for those living in the community were incorrectly reported as living in residential aged care while data for those living in residential aged care were incorrectly reported as living in the community.

9 December 2025 – Text under Measures 8.4–8.5 about a framework for improving dementia data has been updated to align with the updated National Dementia Data Improvement Plan 2023–2034, published 9 December 2025.

• The indicator under Measure 4.8 relating to people with younger onset dementia accessing National Disability Insurance Scheme (NDIS) supports incorrectly referred to the number of participants with an approved NDIS plan, rather than the number of approved plans.
